Pre-Practicum Requirements:
- WSIB Info; U of T memorandum; WEPA forms are available in the Practicum Office
- Police Check:
- Option 1: OESC Card Police Check (link) (check with your instructor if this is acceptable at your site)
- Option 2: Police Check forms available in the Practicum Office - can be taken in person to a police station
- Mask Fitting: By appointment at St. Michael's Hospital; Cost $30, 416-864-6060 ext. 6944, maskfitting@smh.toronto.on.ca Please fill out the N95 Questionnaire and fax/e-mail it to the clinic prior to making an appointment. Instructions are available on the back of the questionnaire.
